Tratamento ortodôntico da agenesia dos incisivos laterais superiores

Abstract

Introdução: A agenesia dos incisivos laterais é uma má formação craniofacial frequentemente encontrada. A ausência de incisivos laterais superiores está na origem de problemas estéticos, oclusais e funcionais. Objetivos: Esta revisão sistemática, tem como objetivo, analisar as possibilidades de tratamento ortodôntico em pacientes com agenesia do incisivo lateral superior permanente. Metodologia: A pesquisa bibliográfica foi realizada na base de dados PubMed, EBSCOhost, AJO-DO, assim como no Google scholar usando as palavras-chave e as suas combinações. Foram reunidos um total de 116 artigos, dos quais 31 foram considerados relevantes. Resultados/ Discussão: Vários casos clínicos mostram as possibilidades de tratamento para diferentes casos da agenesia dos incisivos laterais superiores. As opções de tratamento propostas são abertura de espaço e posterior reabilitação ou fechamento de espaço, posicionando o canino no lugar do lateral. A abstenção do tratamento também pode provocar problemas maiores no futuro. Este trabalho explica os conceitos, as indicações, as vantagens e desvantagens para cada tratamento. Na maioria dos casos, o contexto clínico determina a escolha. Conclusão: O tratamento da agenesia dentária é complexo e requer uma abordagem multidisciplinar incluindo uma análise psicológica exaustiva do paciente. O paciente é decisivo nos critérios de satisfação, da sua motivação e colaboração, e por fim da sua capacidade financeira.

Introduction: Agenesis of lateral incisors is a craniofacial malformation frequently found. The absence of upper lateral incisors is at the origin of aesthetic, occlusal and functional problems.

Objectives: This systematic review aims to analyze the possibilities of orthodontic treatment in patients with permanent maxillary lateral incisor agenesis.

Methodology: The bibliographic search was carried out in the PubMed, EBSCOhost, AJO-DO database, as well as in Google Scholar using the keywords and their combinations. A total of 116 articles were gathered , of which 31 were considered relevant.

Result/ Discussion: Several clinical cases show the possibilities of treatment for different cases of maxillary lateral incisor agenesis. The proposed treatment options are space opening and subsequent rehabilitation or space closing, positioning the canine in place of the lateral. Withholding treatment can also cause greater problems in the future. This work explains the concept, indications, advantages and disadvantages for each treatment. In most cases, the clinical context determines the choice.

Conclusion: The treatment of the tooth agenesis is complex and requires a multidisciplinary approach including an exhaustive psychological analysis of the patient. The patient is decisive in terms of satisfaction, motivation and collaboration, and finally, their financial capacity.
