As proteínas derivadas da matriz do esmalte e a fibrina rica em plaquetas: comparação dos biomaterias na regeneração periodontal
Abstract
A doença periodontal é uma patologia inflamatória da cavidade oral e afeta as estruturas de suporte do dente, originando as bolsas periodontais, com a consequente redução da altura e/ou espessura do osso alveolar.
A evolução tecnológica e a melhor compreensão dos processos biológicos têm levado ao desenvolvimento de diferentes abordagens terapêuticas com o objetivo de corrigir esses defeitos.
Os objetivos desta revisão integrativa são analisar e comparar os resultados de duas combinações regenerativas, compostas por um biomaterial, o derivado da matriz do esmalte ou a fibrina rica em plaquetas, individualmente associados a exerto ósseo autólogo. Consequentemente, são analisados os efeitos fisiológicos que a fibrina rica em plaquetas e o derivado da matriz do esmalte têm sobre os tecidos periodontais. Por fim, são comparados os resultados clínicos que a fibrin rica em plaquetas e o derivado da matriz do esmalte têm sobre a integridade funcional e anatómica, ambos tomados individualmente e tomados em conjunto como um único material obturador.
Foi realizada uma pesquisa bibliográfica a partir em março 2021 na plataforma de busca (bases de dados) PUBMED, COCHRANE, JOURNAL OF PERIODONTOLOGY, RESEARCHGATE, utilizando as palavra-chave: “Regenerative Materials”; “Periodontal regeneration”, “Autogenous bone grafts”, “Enamel matrix derivated”, “Platelet rich fibrin”, que levou à seleção de 24 artigos.
Isoladamente, ambos os biomateriais são válidos para regeneração celular periodontal. A combinação das dois biomaterias leva a melhorias clínicas e radiográficas no tratamento de defeitos intra-ósseos (IBD). Nenhuma vantagem clara é evidente na utilização de osso autólogo com o derivado da matriz do esmalte, em vez em caso do uso de osso autólogo com fibrina rica em plaquetas concordaram com as melhorias clinicas dadas. Periodontal disease is an inflammatory pathology of the oral cavity and affects the tooth's supporting structures, originating periodontal pockets, with the consequent reduction in the height and / or thickness of the alveolar bone.
Technological developments and a better understanding of biological processes have led to the development of different therapeutic approaches in order to correct these defects.
The objectives of this integrative review are to analyze and compare the results of two regenerative combinations, composed of a biomaterial, the enamel matrix derivative or platelet-rich fibrin, individually associated with autologous bone graft. Consequently, the physiological effects that platelet-rich fibrin and the enamel matrix derivative have on periodontal tissues are analyzed. Finally, the clinical results that the platelet-rich fibrin and the enamel matrix derivative have on functional and anatomical integrity are compared, both taken individually and taken together as a single obturator material.

In isolation, both biomaterials are valid for periodontal cell regeneration. The combination of the two biomaterials leads to clinical and radiographic improvements in the treatment of intraosseous defects (IBD). No clear advantage is evident in the use of autologous bone with the enamel matrix derivative, instead in case of using autologous bone with platelet-rich fibrin agreed with the clinical improvements given.
