PRÓTESE SOBRE IMPLANTES: CONEXÃO EXTERNA VS CONEXÃO INTERNA

Abstract

Introdução: Com o desenvolvimento dos implantes, diferentes tipos de conexão dos mesmos à estrutura prótetica, foram desenvolvidos, tais como a conexão externa e interna. Esta revisão integrativa tem como objetivo analisar a relação de algumas complicações biológicas e mecânicas inerentes aos tipos de conexão.

Materiais e Métodos Realizou-se uma pesquisa bibliográfica na base de dados PubMed em Fevereiro de 2021 com um limite temporal dos últimos 10 anos. Obtivemos 683 estudos, dos quais 15 foram analisados.

Resultados: Em geral, existem discrepâncias nos resultados encontrados. No que diz respeito à colonização bacteriana, um estudo encontra maior microfiltração em implantes CE do que os da CI, enquanto outro observa o aumento na CI. Em relação ao hemorragia na sondagem, existem pequenas diferenças significativas, encontrando-se um nível mais alto de sangramento à sondagem para a CI. Quanto à perda óssea, os resultados são contraditórios, alguns autores não encontram diferenças, outros encontram maior perda óssea em implantes CE e outros maior na CI. No que diz respeito ao risco de desaperto/fratura, os resultados também são controversos. A taxa de sobrevivência foi superior a 97% em todos os tipos de conexão avaliados.

Discussão: Para fazer uma comparação fiável quanto ao papel do tipo de conexão, apenas o tipo de conexão tem de ser diferente e manter o resto das características do implante do mesmo. Entre as limitações dos estudos estão que as características da plataforma utilizada não foram constantes, bem como o desenho do implante e o número reduzido de pacientes.

Conclusão: Pode ser sugerido que a escolha do tipo de conexão do implante seja de acordo com as preferências do operador. São necessários mais estudos prospetivos in vivo.

Introduction: With the development of implants, different types of connection of them to the prothetic structure were developed, such as the external and internal connection. This integrative review aims to analyze the relationship of some biological and mechanical complications inherent to the types of connection.

Methods and Materials: For this review, a bibliographical research was carried out using the PubMed database until February 2021. The research identified 683 studies, of which 15 were then analyzed.

Results: In general, there are no discrepancies in the results found. With regard to bacterial colonization, one study finds higher microfiltration in CE implants than CI implants, while another notes the increase in CI. In relation to hemorrhage in the probing, there are small significant differences, with a higher level of bleeding at probing for an CI. Regarding bone loss, the results are contradictory, some authors do not find differences, others find greater bone loss in CE implants and others higher in IC. With regard to risk dand detightening fracture, the results are also controversial. The survival rate was higher than 97% in all types of connection evaluated.

Discussion: To make a reliable comparison of the role of the connection type, only the type of connection has to be different and keep the rest of the implant characteristics the same. Among the limitations of the studies are that the characteristics of the platform used were not constant, as well as the design and material of the implant, as well as a small number of patients.

Conclusion: It may be suggested that choosing the type of implant connection the operator can be according to your preferences. More in vivo foresight studies are needed.
