CORREÇÃO DA MÁ OCLUSÃO DE CLASSE II COM PROMAXILIA COM APARELHOS FUNCIONAIS

Abstract

A Classe II é uma das más oclusões mais comuns encontradas na Medicina Dentária. O tratamento da má oclusão Classe II com promaxilia em pacientes em crescimento são tratados frequentemente com aparelhos funcionais combinados com forças extra orais tendo como objetivo: limitar o crescimento maxilar a mesialização dos molares maxilares, a indução de uma posição anterior da mandibular para a correção do overjet e do overbite conseguindo uma melhoria da relação maxilo-mandibular e do perfil facial. O principal objetivo deste trabalho é conhecer os efeitos esqueléticos, dento-alveolares e no perfil facial, na correção da má oclusão de Classe II com promaxilia em pacientes em crescimento. Foi realizada uma pesquisa bibliográfica nas bases de dados PubMed, Scielo e pesquisa manual usando a combinação das seguintes palavras-chaves: “angle class II”, “headgears”, “functional appliances”, “maloclussion”. Foram selecionados os artigos escritos em português e inglês, num total de 239 artigos, dos quais, foram selecionados 18 que obedeciam os critérios de inclusão definidos. No tratamento da má oclusão de Classe II com promaxilia, foram alcançados bons resultados a nível dento-alveolar e esquelético, com retrusão maxilar, diminuição do SNA e aumento do SNB. Tem-se demonstrado que este tipo de tratamento depende em uma grande parte do momento, padrão de crescimento e colaboração do paciente. Conclui-se que a terapia funcional é eficaz no tratamento de pacientes em crescimento com más oclusões Classe II com promaxilia com melhoria da relação maxilo mandibular e do perfil facial.

Class II is one of the most common malocclusions found in Dental Medicine. The treatment of Class II division I malocclusion in growing patients with functional appliances combined with extra oral forces results in a limitation of maxillary growth, advancement of mandibular structures, increase in lower facial height and correction of overjet and overbite to achieve a good maxillomandibular relationship. The aim of this thesis is to understand the skeletal, dento-alveolar and facial profile effects of Class II malocclusion correction with maxillary prognathism in growing patients with functional appliances combined with extra-oral forces. For this study a search was realized in the PubMed and Scielo databases and manual search using the following combination of terms: "angle class II", "headgears", "functional appliances" and "malocclusion". Articles written in Portuguese and English were selected, with a total of 239 articles, 18 of them were selected because they met the defined inclusion criteria. With the combined treatment of functional appliances and extra-oral forces in a Class II with promaxilia, we obtain good dento-alveolar and skeletal results, maxillary retrusion with a reduction of the SNA angle and mandibular advancement with an increase of the SNB angle. It has been shown that this type of treatment will depend to a great extent on the timing and growth factor of the patients. It is concluded that functional therapy is effective in treating growing patients with Class II malocclusions and in improving the soft tissue facial profile.
