SARPE: impacto esquelético e estético - Revisão sistemática integrativa

Abstract

Introdução A expansão rápida do palato cirurgicamente assistida (ou SARPE) é uma técnica útil para expandir a maxila, ganhar espaço e corrigir as discrepâncias transversais quando o paciente atinge idade de maturidade esquelética.

Objetivo: Avaliar as alterações esqueléticas, dentárias e estéticas com SARPE numa população com maturidade esquelética.

Materiais e métodos: A pesquisa foi realizada nas bases de dados: PubMed, Science Direct e EBSCO host com as seguintes palavras-chave: “Surgically-assisted rapid palatal expansion”, “esthetic changes”, “facial bone changes”, “facial soft tissues”, “skeletal” e “maxillofacial”. A pesquisa eletrónica incluiu artigos entre 2010 e 2022, em inglês.

Resultados: Foram selecionados 10 artigos que cumpriam todos os fatores de inclusão e de elegibilidade. As alterações ao nível do esqueleto facial levam significativamente a uma mudança dos tecidos moles circundante.

Discussão: As alterações nos planos vertical, transversal e sagital influenciam significativamente os tecidos moles circundantes, tais como, cavidade oral, lábios e nariz em particular. As mudanças anatómicas promovem a estética e a harmonia facial do paciente. Conclusão: Existem modificações esqueléticas principalmente ao nível do terço médio e inferior da face, que influenciam os tecidos moles circundantes. O ortodontista tem de levar em consideração estas mudanças e avisar o paciente, no sentido de garantir o consentimento livre e esclarecido destas alterações.

Introduction : Surgically assisted rapid palatal expansion (or SARPE) is a useful technique to expand the maxilla, gain space, and correct transverse discrepancies when the patient reaches an age of skeletal maturity.

Objective: To evaluate skeletal, dental and aesthetic changes after SARPE in a population with skeletal maturity.

Materials and methods: The search was carried out in the following databases: PubMed, Science Direct and EBSCO host with the following keywords: “Surgically-assisted rapid palatal expansion”, “esthetic changes”, “facial bone changes”, “facial soft tissues”, “skeletal” and “maxillofacial”. The electronic search included articles between 2010 and 2022, in English.

Results : 10 articles were selected that met all inclusion and eligibility factors. Changes in the facial skeleton significantly lead to a change in the surrounding soft tissues.

Discussion: Changes in the vertical, transverse and sagittal planes significantly influence the surrounding soft tissues (oral cavity, lips and nose in particular). The anatomical changes promote the patient's facial aesthetics and harmony.

Conclusion: There are skeletal changes mainly in the middle and lower third of the face, which influence the surrounding soft tissues. The orthodontist must take these changes into account and advise the patient, in order to guarantee free and informed consent for these changes.
