Relação entre as disfunções temporomandibulares e a postura da cabeça e da cervical: revisão sistemática integrativa

Abstract

As disfunções temporomandibulares (DTMs) são um grupo heterogêneo de patologias que afetam os músculos envolvidos na mastigação, na ATM ou ambos. Vários autores relataram problemas posturais envolvendo cabeça e cervical também podem causar ou ser causados por DTMs. O objetivo deste trabalho consiste em verificar se existe relação entre DTM, postura da cabeça e cervical. Identificar as opções terapêuticas que podem reduzir os sintomas (fisioterapia, goteira oclusal). Foi realizada uma pesquisa bilbiográfica na Pubmed, com estudos clínicos e randomizados dos últimos 21 anos, tendo-se obtido 14 artigos. 6 estudos concluíram que uma postura anormal da cabeça e da cervical estavam presentes em pacientes com DTM, 4 estudos verificaram que fisioterapia e uso de goteiras são benéficos no tratamento dos pacientes com DTM. O sistema completo que a ATM torna o seu estudo difícil e as justificações científicas complexas por causa das inter-relações entre seus diferentes componentes. De facto, a correlação entre DTM e postura da cabeça e do pescoço parece ser aceite mas a reduzida amostra, riscos de viés, questionário de avaliação da dor e índices torna a avaliação científica subjetiva. No entanto, o uso de goteiras oclusais e da fisioterapia deram melhorias significativas na dor, postura da cabeça e cervical e estabilidade postural na parte superior do corpo em pacientes com DTM. Com este trabalho não foi possível demonstrar cientificamente uma ligação entre DTM e postura da cabeça e do pescoço, embora seja evidente que a utilização das goteiras e a fisioterapia têm impacto significativo no tratamento da DTM.

Temporomandibular dysfunctions (TMDs) are a heterogeneous group of pathologies affecting muscles involved in mastication, temporomandibular joints (TMJs) or both. Several authors reported that postural problems concerning the head and cervical spine can cause or be caused by TMDs. The purpose of this study is to determine whether there is a relationship between TMDs, and head and neck posture, to identify therapeutic options that can reduce the symptoms (physiotherapy, occlusal drip). A bibliographic search was carried out at Pubmed, with clinical and randomized studies from the last 21 years, resulting in 14 articles. 6 studies concluded that abnormal head and neck posture was present in TMDs patients, whilst 4 studies did not. 4 studies found physiotherapy and the use of occlusal splints to be beneficial in treating TMDs patients. The complete system that TMJs are makes its study difficult and its scientific justifications complex because of relationships within its components. In fact, the correlation between TMDs, and head and neck posture seems to be accepted but the small sample sizes, bias risks, pain assessment questionnaire and indices make the scientific assessment subjective and thus were the most limiting factors of the studies. However, the use of occlusal splints and physiotherapy gave significant improvements in neck pain, cervical posture, and overall, postural stability in the upper body in TMD patients. With this work, it was not possible to scientifically demonstrate a link between TMD and head and neck posture, although it is evident that the use of splints and physical therapy have a significant impact on the treatment of TMD.
