Correção da Mordida Aberta Anterior: Grelha palatina removível e fixa convencional vs Grelha palatina suportada por miniparafusos

Abstract

Introdução: As grelhas palatinas atuam como barreira à sucção não nutritiva e, simultaneamente, auxiliam na correção da posição da língua. Recentemente, foram usados miniparafusos para fixar a grelha palatina. Nesta revisão sistemática irão ser comparadas e analisadas as grelhas palatinas já existentes com esta nova. Objetivo: Comparar através da literatura científica, três aparelhos para o tratamento precoce da MAA, as grelhas palatinas removível e fixa convencional com a fixa com miniparafusos. Materiais e Métodos: Formulou-se uma questão PICO segundo as orientações PRISMA. Efetuaram-se pesquisas bibliográficas na base de dados PubMed utilizando combinações de palavras-chave. Resultados: Selecionaram-se 14 artigos dos quais, 4 abordam as técnicas CC, BS, FPC e RPC, 3 descrevem o uso do FPC, 3 comparam o RPC com o FPC, 2 relatam diferenças entre o FPC com o MSPC, 1 descreve as alterações cefalométricas de pacientes em crescimento e 1 avalia a espessura palatina dos tecidos moles e duros, com recurso ao CBCT. Discussão: A grelha palatina removível foi descrita na literatura como sendo muito dependente da cooperação do paciente para a obtenção de bons resultados. Assim, os aparelhos fixos foram investigados como alternativa. A grelha palatina fixa promove a mesialização do primeiro molar superior alterando a relação molar, mas a grelha palatina fixa por miniparafusos apresenta maior taxa de falha, pela pouca densidade óssea em pacientes jovens. Conclusão: Os três aparelhos corretivos descritos proporcionaram uma melhora semelhante no overbite, no entanto, o ideal continua a ser a prevenção e a cooperação entre profissionais de saúde, para evitar o estabelecimento da MAA.

Introduction: Palatal cribs act as a barrier to non-nutritive sucking habits and it also helps correcting the tongue posture. Recently, miniscrews have been used for palatal crib fixation. In this sistematic review, existing palatal cribs were analysed and compared with this new technique.

Objective: Compare, through the scientific literature, three devices for the early treatment of anterior open bite, such as removable and fixed palatal crib with the miniscrews-supported one.

Materials and Methods: A PICO question was formulated according to the PRISMA guidelines. Bibliographic searchs were performed in the PubMed database using combinations of keywords.

Results: 14 articles were selected, 4 of which adress the CC, BS, FPC and RPC techniques, 3 describe the use of FPC, 3 compare the RPC with the FPC, 2 report differences between FPC and MSPC, 1 describes the cephalometric alterations of growing patients and 1 evaluates the thickness of soft and hard tissues, using CBCT.

Discussion: The removable palatal crib was described as being very dependent on the patient's cooperation to obtain good results. Therefore, fixed appliances were investigated as an alternative. The fixed palatal crib promotes mesialization of the maxillary first molar, changing the molar relationship, but the palatal crib fixed by miniscrews has a higher failure rate, due to the low density of the palatine bone in young patients.

Conclusion: The three corrective appliances described provided a similar improvement in overbite, however, prevention and cooperation between health professionals remains the ideal, in order to avoid the establishment of AOB.
