Efficacy of laser therapy in the treatment of Oral Lichen Planus A integrative review

Abstract

Introdução: O líquen plano oral (LPO) é uma doença inflamatória crónica que causa estrias, pápulas ou placas brancas bilaterais na mucosa bucal, língua e gengivas. Eritema, erosões e bolhas podem ou não estar presentes. A laserterapia (por vezes designado por laser terapêutico ou fotobiomodulação com laser) tem sido reportada como uma intervenção útil em várias doenças nomeadamente nos últimos anos em LPO.

Objectivos: Avaliar a eficácia da laserterapia no tratamento do LPO e seus sintomas, analisando se poderá ser um substituto válido para as terapias convencionais ou em combinação com outras intervenções.

Materiais e Métodos: Nesta revisão integrativa, foram pesquisados artigos pertinentes ao estudo nas bases de dados “Pubmed”, "Medline", “ScienceDirect” e “Cochrane Library”. Após a aplicação dos critérios de inclusão e exclusão, 17 artigos foram incluídos no estudo.

Resultados: Todos os estudos relataram efeitos positivos da terapia com laser, especialmente no que respeita ao tratamento dos sintomas de dor. Além disso, muitos estudos sugerem o uso da laserterapia em casos de LPO que não podem ser tratados com os tratamentos convencionais. No entanto, existe uma grande heterogeneidade nos parâmetros do laser utilizados no tratamento do LPO. Para além disso, há uma falta de seguimento prolongado nos estudos para avaliar os efeitos desta intervenção.

Conclusões: A laserterapia tem sido relatada como útil no tratamento dos sintomas de dor e indicada no caso de lesões de LPO que não respondem aos tratamentos convencionais. Estudos adicionais com grupos de controlo e longo tempo de seguimento devem ser promovidos para confirmar estes resultados.

Introduction: Oral lichen planus (OLP) is a chronic inflammatory disease that causes bilateral white striations, papules, or plaques on the buccal mucosa, tongue, and gingivae. Erythema, erosions, and blisters may or may not be present. Laser therapy (sometimes referred to as therapeutic laser or laser photobiomodulation) has been reported as a useful intervention in several diseases, particularly in recent years in OLP. Objectives: To evaluate the effectiveness of laser therapy in the treatment of OLP and its symptoms, analyzing whether if it is a valid substitute for conventional therapies or in combination with other interventions. Materials and Methods: In this integrative review, articles pertinent for the study were searched in Pubmed "Medline", ScienceDirect and Cochrane Library databases. After application of inclusion and exclusion criteria, 17 articles were included in the study. Results: All studies reported positive effects of laser therapy, especially regarding the treatment of pain symptoms. Also many studies suggest the use of laser therapy in cases of OLP that cannot be treated to conventional treatments. However, there is a great heterogeneity in the laser parameters used in the treatment of OLP. Moreover, there is a lack of long follow-up in the studies to evaluate the effects of this intervention. Conclusions: Laser therapy has been reported as useful in the treatment of pain symptoms and indicated in the case of OLP lesions that do not respond to conventional treatments. Further studies with control groups and long follow-up time should be promoted to confirm this results.
