Substituição da estrutura metálica de prótese removível por PEEK. Uma revisão sistemática integrativa

Abstract

Introdução: A Medicina Dentária está em contínua evolução. Encontrar novos materiais que satisfaçam as exigências quer do Médico Dentista quer do paciente, na melhoria das características físicas, mecânicas, estéticas e econômicas requer estudos e testes. A PPR engloba as próteses acrílicas e esqueléticas. Normalmente as PPR esqueléticas são efetuadas em Cr-Co, Cr-Ni, Ti. Objetivos: Avaliar se um material como o PEEK pode ser um bom substituto destas estruturas por não conter metal, ser biocompatível, resistente à radiação ionizante, ter estabilidade química e uma cor esteticamente aceitável. Material e Métodos: Foi realizada uma revisão sistemática integrativa da literatura, para averiguar se o PEEK pode ser um potencial substituto melhorando ou igualando as estruturas metálicas existentes. Para tal foi realizada uma pesquisa bibliográfica na base de dados PubMed utilizando a seguinte combinação de termos de pesquisa: «PEEK» AND «removable prosthesis» , ((polyether ether ketone) AND (metallic)) AND (dental prostheses), (removable partial dentures) and (Peek). Resultados: Este trabalho foi baseado em 13 artigos selecionados e devidamente analisados de acordo com os critérios estabelecidos. Discussão: Através da análise de diferentes artigos foi analisado se o PEEK poderá ser um material de substituição estrutural para as PPR esqueléticas existentes, com características físicas e funcionais adequadas, bem como resultados satisfatórios a curto e longo prazo. Conclusão: Devido à falta de estudos de viabilidade in vivo a longo prazo, é aconselhada a sua limitação a casos de alergias a metais ou casos que requerem uma estética que não é alcançável com ligas metálicas.

Finding new materials that meet the requirements of both the dentist and the patient, in improving physical, mechanical, aesthetic, and economic characteristics requires studies and tests. RPP includes acrylic and skeletal prostheses. Skeletal prostheses are usually made of Cr-Co, Cr-Ni, Ti. Objectives: To assess whether a material such as PEEK can be a good substitute for these structures because it is metal-free, biocompatible, resistant to ionising radiation, chemically stable and aesthetically acceptable in colour. Material and Methods: A systematic integrative literature review was conducted to investigate whether PEEK could be a potential substitute for, or improve on, existing metallic frameworks. For this purpose, a literature search was conducted in the PubMed database using the following combination of search terms: "PEEK" AND "removable prosthesis" , ((polyether ether ketone) AND (metallic)) AND (dental prostheses), (removable partial dentures) and (Peek). Results: This work was based on 13 articles selected and duly analysed according to the established criteria. Discussion: Through the analysis of different articles, it was analysed whether PEEK could be a structural replacement material for existing skeletal RPPs, with adequate physical and functional characteristics, as well as satisfactory short- and long-term results. Conclusion: Due to the lack of long-term in vivo feasibility studies, it is advised to limit it to cases of metal allergies or cases requiring an aesthetic that is not achievable with metal alloys.
