Sinais intra e extraorais de abuso em crianças: implicação forense em odontopediatra

Abstract

Introdução: O abuso e os maus-tratos são classificados como ativos ou passivos. Os maus-tratos ativos são caraterizados pelo abuso físico, emocional, sexual, ou exploração infantil. Os maus-tratos passivos, têm em conta, a noção de negligência geral, psicológica e física. Inclui, também, o conceito de negligência dentária. Objetivo: Estudar os diferentes sinais intraorais e extraorais de maus-tratos em crianças e como o médico dentista os pode detetar no contexto da consulta odontopediátrica. Materiais e métodos: Foi realizada uma pesquisa bibliográfica, nas bases de dados PubMed, Scielo e EBSCO com as palavras-chave: “child abuse”, “pediatric dentistry”, “diagnosis”, “STD”, “Forensic dentistry”, “Forensic odontology” e incluiu artigos entre 2013 e 2023 em inglês, e português. Resultados: Nas pesquisas foram identificados 281 estudos, dos quais 21 foram considerados relevantes para este estudo visto que cumpriam os critérios de elegibilidade. Discussão: Os sinais intraorais compreendem um baixo nível de higiene oral, numerosas lesões de cáries ou de outro tipo de patologia oral, marcas de traumatismo físico ou sexual na cavidade oral. Os sinais extraorais compreendem fatores de risco, sinais comportamentais, marcas de abuso físico e manifestações de doenças sexualmente transmissíveis. Conclusão: Os médicos dentistas não têm por rotina fazer relatórios no caso de suspeita de maus-tratos, por falta de formação e incerteza quanto aos sinais ou por medo de repercussões sobre a criança. É, então, crucial a formação sobre este problema de saúde pública de forma a detetar e diagnosticar corretamente e o mais cedo possível casos de maus-tratos.

Introduction: Abuse and maltreatment is classified as either active or passive. Active maltreatment is characterised by physical, emotional, sexual abuse or exploitation of children. Passive maltreatments are dealing with the notion of general, psychological and physical neglect. It also includes the concept of dental neglect. Objective: To see what are the different intraoral and extraoral signs of child maltreatment, abuse and neglect are and how the dentist can detect them in the context of a dental appointment. Materials and methods: A literature search was conducted, in the PubMed, Scielo and EBSCO databases with the keywords: "child abuse", "pediatric dentistry", "diagnosis", "STD", "Forensic dentistry", "Forensic odontology" and included articles between 2013 and 2023 in English, and Portuguese. Results: In the searches 281 studies were identified, of which 21 were considered relevant that for this study that met the eligibility criteria. Discussion: Intraoral signs include a poor level of oral hygiene, abundance of caries or other types of oral pathologies, the marks of physical or sexual trauma inside the mouth. Extraoral signs include risk factors, behavioural signs, physical abuse marks and manifestations of sexually transmitted diseases. Conclusion: Dentists do not often report suspicious cases because of a lack of knowledge about the signs or fear of repercussions on the child. It is therefore important to be well informed about this public health problem in order to detect as early as possible and with certainty the cases of maltreatment.
