Manifestações orais e tratamentos da mucosite oral nas crianças atingidas de leucemia linfoblástica aguda

Abstract

Introdução: A mucosite oral é um dos efeitos secundários mais prevalentes do tratamento oncológico infantil, na cavidade oral. Pode impactar a qualidade da vida das crianças até ser vítimas de desnutrição e dor exacerbada. Objetivo: Definir e avaliar a eficácia dos meios de prevenção e dos tratamentos, para aliviar a mucosite oral, para as crianças atingidas de leucemia linfoblástica aguda. Materiais e métodos: Foi realizada uma pesquisa eletrónica utilizando a base de dados PubMed. Foram utilizadas combinações de palavras-chave e incluídos artigos publicados entre 2012 e 2023 em inglês e português. Resultados: A pesquisa identificou 60 estudos, dos quais 18 foram conservados para a elaboração deste trabalho Discussão: Têm sido publicados vários tratamentos para tratar e prevenir a mucosite oral. Nesta revisão, vamos abordar algumas destas proposições de tratamento, tais como a leucovorina, a crioterapia oral, a glutamina, a palifermina, o laser, colutórios e algumas soluções naturais. Vamos comparar a eficácia dos tratamentos entre si e elaborar uma proposta de protocolo de higiene oral diário. Conclusões: O tratamento e prevenção das manifestações orais de mucosite oral em crianças com leucemia linfoblástica aguda é um grande desafio, mas melhora a adesão ao tratamento ontológico e permite combater melhor a doença. Além disso, destaca-se a importância de uma comunicação efetiva entre o departamento de oncologia, médicos dentistas e educadores para normalizar os cuidados prestados aos doentes e melhorar os padrões de cuidados orais.

Introduction: Oral mucositis is one of the most prevalent side effects of childhood oncological treatment in the oral cavity. It can have an impact on the quality of life of children and even lead to malnutrition and exacerbated pain. Objective: The primary objective of this systematic review is to define and evaluate the effectiveness of preventive measures and treatments to relieve oral mucositis in children affected by acute lymphoblastic leukemia. Materials and Methods: A research question was formulated, and an electronic search was carried out using the PubMed database. It used combinations of keywords and included articles published between 2012 and 2023 in English and Portuguese. Results: The search identified 60 studies, of which 18 were retained for inclusion in this review. Discussion: Various treatments have been published to treat and prevent oral mucositis. In this review, we will look at some of these treatment proposals, such as leucovorin, oral cryotherapy, glutamine, palifermin, laser, mouthwash, and some natural solutions. We'll compare the effectiveness of these treatments with each other and draw up a proposed daily oral hygiene protocol. Conclusion: Treating and preventing the oral manifestations of oral mucositis in children with acute lymphoblastic leukaemia is a major challenge, enabling them to better accept ontological treatment and better combat the disease. It also highlights the importance of effective communication between the oncology department, dentists, and parents to standardize patient care and improve oral care standards.
