Effectiveness and Predictability of Clear Aligner Appliances Regarding Premolar Rotation: Retrospective longitudinal observational study

Abstract

Introduction: The Invisalign system is increasingly in demand by patients due to its guaranteed aesthetics and good results. However, the efficiency and predictability of movements remain controversial. The rotation of premolars is feasible with this system, but it is one of the most unpredictable and challenging movements.

Objective: To analyse the predictability and accuracy of premolar rotation movement in the permanent dentition using Invisalign orthodontic aligners and to check whether it is facilitated with attachments.

Materials and Methods: Fifty patients were analysed regarding premolar rotation using the clear aligner system, through the Invisalign Doctor Site® platform and Clincheck® Software, comparing the initial moment with the movement predicted and achieved in each refinement. IBM® SPSS® Statistics was employed for statistical analysis. Additionally, a literature search was conducted in the PubMed database.

Results: Statistically significant differences between predicted and achieved rotation were found in all premolars. In rotation between refinements, statistically significant differences were only found between T0-T1 and T1-T2. The use of attachments revealed no significant differences in the effectiveness of rotation.

Discussion: In all the premolars, the movement achieved differed significantly from what was initially predicted. Premolar rotation was effective until the second refinement, after which it became of limited relevance. The use of attachments in this study was not fundamental.

Conclusions: The clear aligner system is effective at rotating premolars, but the predictability of movement is questionable.

Introdução: O sistema Invisalign® é cada vez mais procurado devido à garantia da estética e bons resultados. No entanto, a sua eficiência e previsibilidade de movimentos continua a ser controversa. A rotação de pré-molares é exequível com este sistema, mas é um dos movimentos mais imprevisíveis e desafiantes.

Objetivo: Analisar a previsibilidade e precisão do movimento de rotação de pré-molares na dentição permanente utilizando alinhadores ortodônticos Invisalign® e verificar se a mesma é facilitada com attachments.

Materiais e métodos: Foram analisados 50 pacientes relativamente à rotação de pré-molares com o sistema de alinhadores transparentes, através da plataforma Invisalign Doctor Site® e do Software Clincheck®, comparando o momento inicial, com o movimento previsto e alcançado em cada refinamento. Para a análise estatística foi utilizado o IBM® SPSS® Statistics. Adicionalmente, foi realizada uma pesquisa bibliográfica na base de dados PubMed.

Resultados: Diferenças estatisticamente significativas entre a rotação prevista e a alcançada foram encontradas em todos os pré-molares. Na rotação entre refinamentos apenas se encontraram diferenças estatisticamente significativas entre T0-T1 e T1-T2. A utilização de attachments não revelou diferenças significativas na eficácia da rotação.

Discussão: Em todos os pré-molares, verificou-se que o movimento alcançado diferiu significativamente do previsto inicialmente. A rotação foi efetiva até ao segundo refinamento, a partir do qual se tornou pouco relevante. A utilização de attachments neste estudo não se revelou fundamental.

Conclusão: O Sistema de alinhadores apresenta eficácia na rotação de pré-molares, mas a previsibilidade do movimento é questionável.
