Impacto psicossocial no Lúpus: Imagem corporal, qualidade de vida e autocompaixão

Abstract

O lúpus é uma doença autoimune multifatorial, com um amplo espectro de manifestações clínicas e imunológicas. Esta condição apresenta um grande impacto psicossocial na vida destas pessoas, sendo que o seu tratamento pode ser complexo e difícil. Este estudo tem como objetivo avaliar o impacto psicossocial do lúpus, de forma a contribuir para um maior conhecimento acerca da doença e desenvolvimento de estratégias terapêuticas integradas nas componentes física e psicológica. A amostra inclui 72 pacientes com lúpus, que responderam a um protocolo de avaliação, constituído por um questionário sociodemográfico e clínico, Questionário de Fusão Cognitiva – Imagem Corporal, Índice de Qualidade de Vida – Dermatologia e Escala de Autocompaixão. Foram realizadas estatísticas descritivas de cada instrumento, análises correlacionais e regressões lineares múltiplas. Os resultados demonstram que a qualidade de vida é explicada pelas variáveis: género, alterações relacionais e o impacto na vida social e que pacientes com lúpus que vão a mais consultas de dermatologia recorrem mais a acompanhamento psicológico/psiquiátrico. Este estudo permitiu verificar que variáveis sociodemográficas e clínicas explicam, no seu conjunto, a qualidade de vida nestes pacientes. Além disso, diversas caraterísticas clínicas e aspetos psicológicos, como a fusão cognitiva associada à imagem corporal, qualidade de vida e autocompaixão encontram-se associados ao bem-estar psicossocial destes indivíduos.

Lupus is a multifactorial autoimmune disease with a wide spectrum of clinical and immunological manifestations. This condition has a great psychosocial impact on these people's lives, and its treatment can be complex and difficult, which is why psychosocial interventions are important in order to help patients deal with the different changes of the disease. This study aims to assess the psychosocial impact of lupus, in order to contribute to greater knowledge about the disease and the development of therapeutic strategies integrated in the physical and psychological components. The sample includes 72 patients with lupus, who responded to an evaluation protocol, consisting of a sociodemographic and clinical questionnaire and the instruments Cognitive Fusion Questionnaire – Body Image, Quality of Life Index – Dermatology and Self-Compassion Scale. Descriptive statistics for each instrument, correlational analyzes using Pearson's correlations for the total scales and subscales and Spearman correlations were performed to assess the relationship between the total scales and subscales and clinical and sociodemographic variables and multiple linear regressions, with the aim of finding dependent variables that explained the quality of life in this sample. The results show that quality of life is explained by the variables: gender, relational changes and the impact on social life and that patients with lupus who attend more dermatology consultations resort more to psychological/psychiatric follow-up. This study allowed us to verify which sociodemographic and clinical variables explain the quality of life in these patients. In addition, several clinical characteristics and psychological aspects, such as cognitive fusion associated with body image, quality of life and self-compassion are associated with the psychosocial well-being of these individuals.
