Alterações gengivais em pacientes tratados ortodonticamente

Abstract

Introdução: O tratamento ortodôntico estabelece uma oclusão funcionalmente e esteticamente estável através de movimentos dentários adequados. Devido à complexidade das reabilitações estéticas e funcionais, a associação dos tratamentos periodontais e ortodônticos é necessária para atingir os objetivos do tratamento. Previamente ao tratamento ortodôntico deve ser realizada uma avaliação do biótipo gengival que contribui para um diagnóstico clínico apropriado. Objetivos: Verificar o comprometimento dos tecidos periodontais em pacientes tratados ortodonticamente e analisar a relação entre o biótipo gengival e a terapia ortodôntica. Materiais e Métodos: Efetuaram-se pesquisas bibliográficas na base de dados Pubmed com as seguintes palavras-chave: "“Periodontal Diseases”, “Orthodontics”, “Tooth Movement”, “Gingival Recessions” e “Gingival Biotype”, no intervalo de tempo entre 2013-2023. Resultados: De acordo com a expressão de pesquisa avançada foram identificados um total de 128 artigos. A pesquisa bibliográfica dos últimos 10 anos, com idioma diferente de português ou inglês resultou em 52 artigos. Após a seleção de artigos acessíveis ao abstrato ficaram 42 artigos. Com base na relevância do título foram excluídos 30 artigos, incluindo 19 artigos finais, sendo que 7 desses artigos foram adicionados de forma manual. Discussão: Foi possível averiguar que o tratamento ortodôntico em conjunto com a colaboração do paciente e com a inexistência de inflamação periodontal, pode induzir a resultados satisfatórios sem causar danos irreversíveis para os tecidos periodontais. Dessa forma, o mesmo pode ainda aumentar as hipóteses do tratamento periodontal em determinados pacientes. Conclusão: Pacientes com biótipo gengival fino são considerados de alto risco para a exposição da superfície radicular.

Introduction: Orthodontic treatment establishes a functionally and aesthetically stable occlusion through proper tooth movement. Due to the complexity of esthetic and functional rehabilitations, the association of periodontal and orthodontic treatments is necessary to achieve the treatment goals. Prior to orthodontic treatment, an assessment of the gingival biotype should be performed, which contributes to an appropriate clinical diagnosis.

Objectives: To verify the involvement of periodontal tissues in patients treated orthodontically and analyze the relationship between gingival biotype and orthodontic therapy.

Materials and Methods: Literature searches were performed in the Pubmed database with the following keywords: "Periodontal Diseases," "Orthodontics," "Tooth Movement", "Gingival Recessions" and “Gingival Biotype” in the time range 2013-2023.

Results: According to the advanced search expression, a total of 128 articles were identified. The bibliographic search of the last 10 years, with language other than Portuguese or English resulted in 52 articles. After the selection of articles accessible to the abstract, 42 articles remained. Based on title relevance 30 articles were excluded, including 19 final articles, and 7 of these articles were added manually.

Discussion: It was possible to verify that orthodontic treatment in conjunction with patient compliance and the absence of periodontal inflammation, can lead to satisfactory results without causing irreversible damage to periodontal tissues. Thus, it can also increase the chances of periodontal treatment in certain patients.

Conclusion: Patients with thin gingival biotype are considered at high risk for root surface exposure.
